  1. To MS-DOS 6.0/DoubleSpace Users,
  2.    My name is Bill Golden.  I am a Certified Public Accountant in Roswell, 
  3. New Mexico.  I have been retained to gather information about users who 
  4. may have experienced problems in implementing and using MS-DOS 6.0 with 
  5. DoubleSpace.  The information gathered is to be used, if warranted, as the 
  6. basis for a class-action lawsuit against Microsoft Corp.
  7.    If you encountered major problems and/or data loss in your implementa-
  8. tion of MS-DOS 6.0 and DoubleSpace, we want to hear from you, even if you 
  9. eventually resolved the error/problem by removing DoubleSpace.
  10.    It is not my intent to determine the extent or nature of technical 
  11. deficiencies of MS-DOS 6.0/DoubleSpace, if any.  I have reviewed documen-
  12. tation where Microsoft offered one DoubleSpace user $5,000 of Microsoft 
  13. product in exchange for his releasing Microsoft of any responsibility with 
  14. problems encountered in his implementation of DoubleSpace.
  15.    Microsoft may have offered similar settlements to one or more users.  If 
  16. you were offered AND ACCEPTED any settlement with Microsoft regarding prob-
  17. lems with MS-DOS 6.0 and DoubleSpace, you should not respond to this survey.

  25.    All replies will be held strictly confidential.  Should the information 
  26. gathered be determine sufficient to warrant the filing of a class-action 
  27. lawsuit, you will be directly notified if you responded to the survey.

  29. 1. Your name, address and phone number?
  30. 2. Approximate date and place you purchased MS-DOS 6.0?
  31. 3. Description of the computer(s) on which you implemented MS-DOS 6.0 and 
  32.    DoubleSpace? (example: Intel 386, 130 Meg Seagate HD, 4 meg of memory, 
  33.    running SmartDrive, etc.)
  34. 4. What Disk Operating System where you running just prior to your imple-
  35.    mentation of MS-DOS 6.0 and DoubleSpace?
  36. 5. Is the computer you implemented MS-DOS 6.0 with DoubleSpace on used 
  37.    primarily for business or personal use?
  38. 6. Briefly describe the problems you encountered with DoubleSpace?
  39. 7. Please estimate the number of man-hours expended by you or others in 
  40.    resolving the problems you encountered with DoubleSpace, even if the 
  41.    resolution of the problem was to remove DoubleSpace?  Include any time 
  42.    expended on your part in reloading and/or re-configuring other applica-
  43.    tion software damaged as a consequence of your implementation of Double-
  44.    Space.
  45. 8. Please describe any software or software product you lost and were 
  46.    unable to retrieve as a consequence of your implementation of Double-
  47.    Space?  Please value each application item separately.  With respect 
  48.    to software product produced by your own efforts, please estimate the 
  49.    approximate number of man-hours expended in creating the product, 
  50.    compiling the data, etc.
  51. 9. If you sought the assistance of Microsoft Technical Support, please 
  52.    describe what steps, if any, they instructed you to undertake in 
  53.    resolving the problem?
